These questionnaires intend to find out about your general health and wellbeing 6 and 12 months after your surgery - the questions are not specific to the operation you had. As such, please answer the questions in a general manner, telling us how you feel overall on the day you are completing the questionnaire, taking any other health conditions into account.
Your answers will be saved as you progress through each page of the questionnaire, allowing you to exit the questionnaire and return to it later if you wish. However, once you have saved and submitted your questionnaire at the end, the answers will appear greyed out and you will not be able to change them.
